In practice, the period after active cancer treatment ends often brings its own distinct set of questions and challenges: what follow-up testing is actually needed, which symptoms should prompt concern, how to manage lingering physical or emotional effects, and how to coordinate care between an oncologist and a primary care doctor going forward. A survivorship care plan is a structured document designed to answer exactly these questions.
Despite growing recognition of their value, survivorship care plans are not yet offered consistently across every cancer center, and many survivors are unaware such a document exists or that they can specifically ask for one.
