American Oncology Institute (AOI) Mangalagiri in Vijayawada Saves 48-Year-Old Man’s Leg from near Amputation

Successful Reconstructive Plastic Surgery Using Anterolateral Thigh (ALT) Free Tissue Transfer

Vijayawada, 04th Sept 2023: American Oncology Institute (AOI) Mangalagiri in Vijayawada is successful treated a 48-year-old male, diagnosed with an ulceroproliferative lesion over his right thigh. An ulceroproliferative lesion refers to an abnormal area on the skin that has developed into an open sore (ulcer) and is growing or spreading. The condition had been troubling the patient for six months.

The patient had a history of a burn injury to the right thigh, which resulted in a burn scar. The lesion began as a small ulcer and gradually expanded to an alarming size of 11x9x8 cm. Mr. Rana Raghavendra (name changed) had been advised for above-knee amputation at other hospitals, due to the size of the lesion but sought a second opinion at American Oncology Institute (AOI) Mangalagiri in Vijayawada.

Upon evaluation, a PETCT scan revealed a localized lesion on the right thigh with no evidence of distant metastasis. The medical team led by Dr Kalyan Chakradhar Polavarapu, Surgical Oncologist at AOI consulted with the plastic surgeon for reconstruction options. The main challenge was the limited choice for local flap cover due to the presence of a burn contracture surrounding the lesion. After careful consideration, it was decided to proceed with a free Anterolateral Thigh (ALT) flap from the opposite thigh. Mr. Rana underwent a wide excision of the lesion over the right thigh, followed by a free ALT flap reconstruction harvested from the left thigh. Remarkably, the patient was discharged on the 8th post-operative day.

Dr Kalyan Chakradhar Polavarapu, Surgical Oncologist, AOI Vijayawada, highlighted the significance of this accomplishment, stating, "This case presented a unique set of challenges due to the extensive burn scar tissue. However, our team's dedication to delivering patient-centered care and exploring innovative solutions led us to preserve the patient's limb while successfully treating a large squamous cell carcinoma of the leg. This outcome exemplifies our commitment to pushing the boundaries of medical care to improve the lives of our patients."

Mr. Mahendra Reddy, RCOO, American Oncology Institute, Vijayawada, expressed his gratitude in the contribution to this remarkable achievement. He said, "At AOI, our mission is to deliver world-class cancer care with compassion and innovation, while our vision is to transform the landscape of cancer care by constantly pushing the boundaries of what is possible.
